We found a bolt on the floor and later realised that the seat base was loose. The bolt fitted the bracket in the seat. We checked the other 3 brackets and one was nearly out and the other 2 were loose.
Checking the bolts are tight is worth adding to your maintenance routine

I had the same problem. I can see why it happens - it's meant to allow movement so it isn't tight, but the nut vibrates loose eventually. I replaced mine with a nylon threaded nut to avoid this in future.

There was a specific plastic washer in mine, i assume to avoid the metal plates touching and assist movement.
